    Influence of the inlet air temperature on the microencapsulation of kenaf (Hibiscus cannabinus L.) seed oil.

    The aim of this study was to evaluate the influence of different inlet air temperatures on the physicochemical properties and oxidative stability of microencapsulated kenaf seed oil (MKSO). Kenaf seed oil was homogenised with the wall materials at a total solid content of 30% and was spray-dried at 160, 180 or 200°C inlet air temperature. The microstructure and morphology of the microencapsulated kenaf seed oil were observed using a scanning electron microscope. The physicochemical properties, such as moisture content, water activity and particle size, of MKSO produced at different inlet air temperatures showed a significant difference (p<0.05). MKSO produced with an inlet air temperature of 160°C exhibited the highest microencapsulation efficiency (MEE, 96.46%) compared to 180°C (78.42%) and the efficiency was lowest at 200°C (58.96%). Increasing the inlet air temperature also resulted in significantly increased (p<0.05) lipid oxidation of MKSO and decreased total intrinsic phenolic content upon accelerated storage. However, all MKSO had lower lipid oxidation and higher total phenolic content than bulk (unencapsulated) oil. This study indicates that increased inlet air temperature results in larger particle size, higher lipid oxidation and lower MEE. The process of microencapsulation could protect oil from the external environment that causes lipid oxidation. Practical applications: Kenaf seed oil contains PUFA and phytosterols, which are beneficial to human health. However, the PUFA in kenaf seed oil is susceptible to lipid oxidation, which degrades its nutritional value. Microencapsulation is used to protect the kenaf seed oil from being oxidised. By knowing the influence of the inlet air temperature on the physical properties and oxidative stability of the microencapsulated kenaf seed oil, the ideal inlet air temperature can be used to produce microencapsulated kenaf seed oil, which may be incorporated into food products to supplement the bioactive compounds that are beneficial to human health

    Protective actions of des-acylated ghrelin on brain injury and blood-brain barrier disruption after stroke in mice

    The major ghrelin forms, acylated ghrelin and des-acylated ghrelin, are novel gastrointestinal hormones. Moreover, emerging evidence indicates that these peptides may have other functions including neuro- and vaso-protection. Here, we investigated whether post-stroke treatment with acylated ghrelin or des-acylated ghrelin could improve functional and histological endpoints of stroke outcome in mice after transient middle cerebral artery occlusion (tMCAo). We found that des-acylated ghrelin (1 mg/kg) improved neurological and functional performance, reduced infarct and swelling, and decreased apoptosis. In addition, it reduced blood-brain barrier (BBB) disruption in vivo and attenuated the hyper-permeability of mouse cerebral microvascular endothelial cells after oxygen glucose deprivation and reoxygenation (OGD + RO). By contrast, acylated ghrelin (1 mg/kg or 5 mg/kg) had no significant effect on these endpoints of stroke outcome. Next we found that des-acylated ghrelin's vasoprotective actions were associated with increased expression of tight junction proteins (occludin and claudin-5), and decreased cell death. Moreover, it attenuated superoxide production, Nox activity and expression of 3-nitrotyrosine. Collectively, these results demonstrate that post-stroke treatment with des-acylated ghrelin, but not acylated ghrelin, protects against ischaemia/reperfusion-induced brain injury and swelling, and BBB disruption, by reducing oxidative and/or nitrosative damage

    A preliminary study: Does relationship closeness with grandchildren correlate with the quality of life and physical health of Malaysian Chinese elderly?

In the quantitative study, a total of 97 Chinese grandparents who are primary caregivers were recruited to examine the relations of relationship closeness, quality of life and physical health. Correlational analysis revealed that grandparent-grandchildren relationship closeness positively correlated with self-rated quality of life and physical health of the grandparents. In the qualitative interview, grandmothers (n = four Chinese grandmothers) who are primary caregivers were recruited to share their experience in taking care of grandchildren. Findings from the thematic analysis revealed that all grandmothers have a close relationship with their grandchildren. In specific, they are happy with the companionship of grandchildren even though they may feel physically tired in taking care of grandchildren. This preliminary study on skipped generation family provides insights into understanding the contribution of relationship closeness with grandchildren on the perceived quality of life and physical health of Malaysian grandparents who are the primary caregiver to their grandchildren

    A Novel Respiratory Rate Estimation Algorithm from Photoplethysmogram Using Deep Learning Model

    Respiratory rate (RR) is a critical vital sign that can provide valuable insights into various medical conditions, including pneumonia. Unfortunately, manual RR counting is often unreliable and discontinuous. Current RR estimation algorithms either lack the necessary accuracy or demand extensive window sizes. In response to these challenges, this study introduces a novel method for continuously estimating RR from photoplethysmogram (PPG) with a reduced window size and lower processing requirements. To evaluate and compare classical and deep learning algorithms, this study leverages the BIDMC and CapnoBase datasets, employing the Respiratory Rate Estimation (RRest) toolbox. The optimal classical techniques combination on the BIDMC datasets achieves a mean absolute error (MAE) of 1.9 breaths/min. Additionally, the developed neural network model utilises convolutional and long short-term memory layers to estimate RR effectively. The best-performing model, with a 50% train–test split and a window size of 7 s, achieves an MAE of 2 breaths/min. Furthermore, compared to other deep learning algorithms with window sizes of 16, 32, and 64 s, this study’s model demonstrates superior performance with a smaller window size. The study suggests that further research into more precise signal processing techniques may enhance RR estimation from PPG signals

    Protein expression patterns in HEK-blue-cells treated with Clinacanthus nutans extracts

    Background: Clinacanthus nutans (CN) is a small shrub native to tropical Asia known for their anti-oxidant, anti-inflammation, anti-cancer, and anti-viral activities. Objectives: This study aimed to investigate the effect of CN extract on human embryonic kidney cell line (HEK-Blue™-4) in a proteomic perspective. Materials and Methods: Comparative proteomic profiling through two-dimensional sodium dodecyl sulfate gel electrophoresis was performed on HEK-Blue™-4 treated with CN leaf polar extract. Results: We successfully identified seven upregulated proteins, of which five promoted the growth of the HEK-Blue™-4 cells. Interestingly, a potent antioxidant enzyme which neutralizes reactive oxygen or nitrogen species, peroxiredoxin-1 was also upregulated in HEK-Blue™-4 cell lines after treatment with CN leaf polar extract. Conclusion: CN leaf polar extract promotes the growth of HEK-Blue™-4 cells and induced the expression of peroxiredoxin-1, which protects the cells from reactive oxygen species during the inflammation process

    Pulse wave velocity is associated with increased plasma oxLDL in ageing but not with FGF21 and habitual exercise

    Fibroblast growth factor 21 (FGF21) and adiponectin increase expression of genes involved in antioxidant pathways, but their roles in mediating oxidative stress and arterial stiffness with ageing and habitual exercise remain unknown. We explored the role of the FGF21–adiponectin axis in mediating oxidative stress and arterial stiffness with ageing and habitual exercise. Eighty age- and sex-matched healthy individuals were assigned to younger sedentary or active (18–36 years old,n=20 each) and older sedentary or active (45–80 years old,n=20 each) groups. Arterial stiffness was measured indirectly using pulse wave velocity (PWV). Fasted plasma concentrations of FGF21, adiponectin and oxidized low-density lipoprotein (oxLDL) were measured. PWV was 0.2-fold higher and oxLDL concentration was 25.6% higher (both p<0.001) in older than younger adults, despite no difference in FGF21 concentration (p=0.097) between age groups. PWV (p=0.09) and oxLDL concentration (p=0.275) did not differ between activity groups but FGF21 concentration was 9% lower in active than sedentary individuals (p=0.011). Adiponectin concentration did not differ by age (p=0.642) or exercise habits (p=0.821). In conclusion, age, but not habitual exercise, was associated with higher oxidative stress and arterial stiffness. FGF21 and adiponectin did not differ between younger and older adults, unlikely mediating oxidative stress and arterial stiffness in healthy adults. <br

    Prevalence, awareness and control of hypertension in Malaysia from 1980-2018: a systematic review and meta-analysis

    Background: Hypertension is a common public health problem worldwide and is a well-known risk factor for increased risk of cardiovascular diseases, contributing to high morbidity and mortality. However, there has been no systematic review and meta-analysis of a multiethnic population such as that of Malaysia. Aim: To determine the trend in prevalence, awareness and control rate of hypertension in Malaysia. Methods: Systematic searches were conducted in six databases (PubMed, Scopus, Ovid, CINAHL, Malaysian Medical Repository and Malaysia Citation Index) for articles published between 1980 and 2018. Two authors reviewed the studies and performed quality assessment and data extraction independently. Pooled estimates of hypertension prevalence, awareness and control rate were calculated using the DerSimonian-Laird random-effects model. Subgroup and sensitivity analyses were performed. Results: We included 56 studies involving a total of 241796 participants. The overall pooled prevalence of hypertension aged ≥ 18 years was 29.7%. The prevalence of hypertension was the lowest in the 1980s (16.2%, 95% confidence interval (CI): 13.4, 19.0), increasing up to 36.8% (95%CI: 6.1, 67.5) in the 1990s, then decreasing to 28.7% (95%CI: 21.7, 35.8) in the 2000s and 26.8% (95%CI: 21.3, 32.3) in the 2010s. The prevalence of awareness was 51.4% (95%CI: 46.6, 56.3), while 33.3% (95%CI: 28.4, 38.2) of those on treatment had achieved control of their blood pressure. Conclusion: In Malaysia, three in ten adults aged ≥ 18 years have hypertension, while four in ten adults aged ≥ 30 years have hypertension. Five out of ten people are aware of their hypertension status and only one-third of those under treatment achieved control of their hypertension. Concerted efforts by policymakers and healthcare professionals to improve awareness and control of hypertension should be of high priority

    A cross-sectional study on level of knowledge and level of attitude towards asthma among the sampled population aged 13 years and above of Rumah Uming and Rumah Cherida, Meradong, from 20th November 2006 to 26th January 2007

    Introduction: Asthma is a chronic inflammatory disorder of the airways characterized by wheezing, breathlessness, chest tightness and cough. Asthma affects people of all ages. In Malaysia, asthma was among the commonest conditions that accounted for outpatient department attendance. Asthma is not curable, yet an asthmatic patient can live a normal life ifit is well-controlled. Objectives: The objective of this study is to study the level of knowledge and level of attitude towards asthma among the sampled population aged 13 years and above in Rumah Uming and Rumah Cherida from 20th November 2006 to 26th January 2007. Methodology: A cross-sectional study on level of knowledge and level of attitude towards asthma was carried out. The sample size of 90 respondents was chosen using a simple random sampling method. Data collection was done by means of interview-guided questionnaire, and all collected data was entered and analysed using SPSS software version 13. The statistical tests that were used include Pearson correlation test, chi-square test, independent sample t-test, ANOV A test, Spearman rho test, Mann-Whitney U test and Kruskal-Wallis test. Results: The main source of information regarding asthma was obtained from mass media. The study showed more than half of the respondents had poor level of knowledge in terms of general knowledge and signs and symptoms (62.2%), risk factor and triggering factor (55.6%), and treatment and prevention of asthma (60.0%). However, 56.7% of respondents had good knowledge about complications of asthma. There was a significant relationship between total level of knowledge with gender (p = 0.034), household income (p = 0.018), and personal and family history of asthma (p = 0.008). Females, higher income group and respondents with personal and family history of asthma showed better score of knowledge. Most of the respondents had good attitude in terms of seeking knowledge (65.6%) and prevention of asthma (53.3%), however 55.6% showed poor attitude in terms of the treatment of asthma. Significant association was found between total level of attitude and level of education (p = 0.042). There was also a positive relationship between the total level of knowledge and the total level of attitude towards asthma among the respondents (p = 0.023). Conclusion: The study revealed a significantly higher level of knowledge among the respondents who were female, with higher household income and with personal and family history of asthma

    Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.
